Intake and Assessment Specialist

JOB SUMMARY:

The Intake & Assessment Specialist is responsible to conduct initial assessment and screening of individuals to determine eligibility, placement, and acuity of presenting issues. This position initiates the process for determining eligibility of individuals whom present for admission at the facility. As the first clinical contact with agency, this individual must promote client engagement, dignity, and respect. In a diagnostic capacity, make the preliminary diagnosis and accurate documentation of client assessments, screening, and needs.

ESSENTIAL JOB FUNCTIONS:

  1. Conducts assessments or screenings with clients to evaluate the nature of their presenting problems in order to determine their needs.
  2. Has responsibility for intake documentation and coordination with the admission and medical department.
  3.  Conduct the initial assessment process, to include but not limited to;
    1.  An initial plan for treatment; i. Indicating the emergent needs and evaluation of the individual
    2. ASAM Biopsychosocial and/or Locus Scoring Assessment
    3. SAFE-T and when indicate, Suicide Risk Assessment (SRA) and develop, with the client, a plan of safety in moderate or high risk for presenting suicide potential.
    4. Trauma Screens (PCL-5, BTQ, and/or ACES)
    5. Pain Screening
    6. Cultural and Spiritual Screening
    7. HRSN Screening (PRAPARE)
    8. When applicable, Educational Assessment, Legal Assessment,
    9. Develop the Problem List, behavioral evidence, and reconcile screening and assessment process in the ‘problem list’
  4. Collect client history of presenting illness and conduct comprehensive assessment
  5. Responsible for the screening and assessment of client admissions to determine the care, treatment, or services that will best meet the needs of the individual served initially and over time.
  6. Accurately identifying the needs of the individual:
    1. Collecting data about the individual’s current and past emotional and behavioral functioning, needs, strengths, preferences, and goals.
    2.  Analyzing data to produce information about the individual’s need for care, treatment, or services and to identify the need for additional data.
    3. Making care, treatment, or service decisions based on the information developed about the needs, strengths, preferences, and goals of the individual served and their response to care, treatment, or services.
  7. Evaluate criteria to determine eligibility for care, treatment, or services; initial placement and coordination with medical department to meet the needs of the individual.
  8. Determine the need for further screening/assessment is determined by the care treatment, or services sought; the individual’s presenting condition(s); and whether the individual agrees to care, treatment or services.
  9. Screen for the early detection of risk of imminent harm to self or others.
  10. Adhere to timeframes, placement, and procedural compliance with Cornerstone Healing Center’s policies and procedures.
  11. Completes other tasks as assigned.
