JOB SUMMARY:
Under the general direction of the agency’s Clinical Director, the Case Manager is responsible for providing direct patient care under the clinical supervision of the Clinical Director. The Case Manager also maintains a safe and efficient working and treatment environment per company policies, procedures and best practices; communicates effectively with the treatment team to ensure safe, quality care is provided to all patients.
ESSENTIAL JOB FUNCTIONS:
- Accomplishes clients’ care by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches; monitoring staff performance.
- Admits new clients by reviewing records and applications; conducting orientations, coordinating medical record requests from previous providers.
- Determines clients’ requirements by completing intake interviews; determining need for therapeutic medical, psycho-social, and psychiatric evaluations; reviewing therapist evaluations, treatment objectives, and plans in connection with relapse prevention plans and continuing care referral.
- Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation and escort.
- Monitors cases by verifying clients’ attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support.
- Maintains clients’ records by reviewing case notes; logging events and progress.
- Communicates clients’ progress by conducting weekly interdisciplinary meetings and evaluations; disseminating results and obstacles to therapeutic team and family; identifying treatment influences.
- Prepares clients’ discharge by reviewing and amplifying discharge plans; coordinating discharge and post-discharge requirements; orienting and training family members; providing resources.
- Improves staff competence by providing educational resources; balancing work requirements with learning opportunities; evaluating the application of learning to changes in treatment results.
- Conducts client surveys, including outcome surveys, satisfaction surveys, and other tools as directed.
- Assist clients in creating budget by monitoring expenses; implementing cost-saving actions.
- Prepares reports by collecting, analyzing, and summarizing treatment and results data and trends; compiling statistics; completing grant and subsidy applications.
- Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations; engaging clients for resume building and job training.
- Enhances department and organization reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.
- Facilitates group as directed, with bi-weekly supervision of clinical director.
- Conducts COVID Screening/questionnaire by phone or interview; takes client temperature and follows infection control procedures.
ESSENTIAL FUNCTION STATEMENT

EDUCATION AND EXPERIENCE
- Case Manager Meets qualifications of a BHT
a.An associate’s degree (18 credit hours specific to BH), and two years full time in the BH field, or
b. A high school diploma, and four years full time in the field, or
c. A high school equivalency diploma, and four years full time in the field, or
d. A College Degree and 1-year full time in the field, or
e. A Master’s degree and 1-year full time in the field.
KNOWLEDGE, ABILITIES, AND ATTITUDES REQUIRED
- Knowledge of software programs for personal computer use, especially word processing, email, and spreadsheets.
- Knowledge and experience in 12 step program and with substance abuse patients.
- Strong problem-solving skills and the ability to carry out assigned projects to their completion.
- Strong interpersonal skills, including the ability to communicate effectively both verbally and in writing, establishing and maintaining effective working relationships, gaining cooperation and resolving conflicts.
- Ability to plan and organize case workload, manage deadlines of him/herself.
- Ability to maintain and demonstrate ethical professional standards and demeanor, including the confidentiality of employee and patient data, patient boundaries, and other sensitive information.
- Demonstrated competence with, and continued commitment to respect for cultural differences and diversity, protecting patient rights.
- Recognition of the limits of one’s own professional expertise and the willingness to seek supervision and/or support, as needed.
- Makes clinical and organizational decisions with the patients’ and agency well-being and best interests in mind.
- Provide treatment that promotes patient dignity, independence, individuality, strength, privacy and choice, recognizing the unique needs of the patient population served by the agency.
- Provide the behavioral health services that the agency is authorized to provide and that the staff member is qualified to provide.
- Recognize obvious symptoms of a mental disorder, personality disorder, or substance abuse.
- Identify types of medications commonly prescribed for mental disorders, personality disorders and substance abuse, and the common side effects and adverse reactions to medications.
- Recognize, prevent and respond to a situation in which the patient may be a danger to himself or others, behaves in an aggressive or destructive manner, is experiencing a crisis situation or medical emergency.
- Recognize and respond to fire, disaster, hazard, and medical emergency: and
- Has completed all of the requirements above and is documented in application in training including the ability to pass background checks required by Cornerstone Healing Center, LLC.
